Remote Patient Monitoring

Continuous monitoring using FDA-cleared medical devices. Track vital signs and health metrics remotely.

What is Remote Patient Monitoring?

Remote Patient Monitoring (RPM) enables healthcare providers to monitor patients' physiological data remotely using FDA-cleared medical devices. This program is ideal for patients with chronic conditions who benefit from continuous monitoring.

Key Features

  • FDA-cleared device management and assignment
  • Automatic reading tracking (≥16 days/month requirement)
  • Clinical staff time management (≥20 minutes/month)
  • Patient communication logging
  • Medicare compliance validation
  • CPT Codes: 99453, 99454, 99457, 99458, 99473, 99474

Who Can Benefit?

RPM is ideal for patients who:

  • Have chronic conditions requiring continuous monitoring
  • Need to track vital signs regularly (BP, glucose, weight, etc.)
  • Would benefit from early intervention alerts
  • Prefer monitoring from the comfort of home

How It Works

Patients are provided with FDA-cleared medical devices (blood pressure monitors, glucose meters, weight scales, etc.) that automatically transmit readings to their care team. The care team monitors these readings and provides timely interventions when needed.

Medicare covers RPM services when patients meet the requirements of at least 16 days of readings per month and 20 minutes of clinical staff time.
